Patient Participation Week 10th - 15th June - The Patient Group hopes that you will pop into the surgery during this week to pick up their Newsletter and they are now accepting new members to their group so there will be forms available, enabling you to join the group.

Car Park Barrier - The barrier will be open during normal Surgery hours; 8am to 6.30pm Monday to Friday and 9am to 11.30am on Saturdays. If your car is in the car park outside of these times you will be able to exit the car park but not enter, however, it is your responsibility to ensure that your car is not left in the car park outside of our normal opening hours as we do not accept any liability if your car is in the car park and we have a barrier electrical failure outside of our normal opening hours.

CAR PARK - The Car Park resurfacing has now been completed and the car park is now open for use - we apologise that the white lines have yet to be added and the disabled spaces marked.

New Patient Registration

If you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure. Eligibility can be quickly confirmed from your address so please provide proof by way of a recent utility bill. We also require either a passport or birth certificate.

Proof of Identity requirements

You will need to complete a registration form (GMS1) and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor.

REPEAT MEDICATIONS

All new patients on repeat medications must have an appointment with our pharmacist prior to being issued with any repeat medications, this appointment can be made at time of registering, please ensure that you have sufficient medication from your previous surgery prior to moving to us as we unable to issue repeat prescriptions for new patients until after the pharmacist appointment.
